HOW DO I GET SERVICES?
In order for us to help you, first register with the State Dept. of Health, through the mental health “Access Line.” This means you will agree to give the Adult Mental Health Division your personal health information, so they may see if you qualify for services. The worker will explain this to you. When you phone Access Line, they will set up an appointment for you to speak to an evaluator. Show them this brochure–tell the evaluator that you want case management services from Community Empowerment Resources. Mental Health Access Line on Oahu: (808) 832-3100.
A DIFFERENT KIND OF AGENCY
Community Empowerment Resources (CER) specializes in Community-Based Recovery Management. This means providing services that support the recovery of adults with mental illness, to further their personal recovery goals. Our qualified staff has a wealth of experience. Our goal is to help you and give you guidance as long as you desire it. Need housing? A job? We’re here to help.
Importantly, CER employees and consumers have input into how services are delivered. Your suggestions are taken seriously at CER.
CER’s MISSION STATEMENT
“Community Empowerment Resources values its consumers and employees by empowering them to live happy and fruitful lives. Consumers receive services founded on Evidence-Based, culturally sensitive, warm and welcoming, individualized person-centered services in the community, in spite of the presence of a disability. Building on each consumer’s and each employee’s individual strengths produces high-quality health care and social services and establishes an effective and healthy agency.
HOMELESS/EMPLOYMENT TEAM FOCUS
We help all consumers eligible for services through the Adult Mental Health Division (of the Hawaii State Dept. of Health). Our primary focus is on helping Homeless consumers, and on helping consumers find and keep competitive employment.
We follow the “Recovery Model”, which is a national set of standards for helping people with mental illness. The recovery model has been proven to work; it states that things are done WITH the person being served, not TO the person. There are five main principles in the recovery model: Hope, Purpose, Acceptance, Mutual Help, and Responsibility.
CER PROVIDES YOU WITH A “RECOVERY TEAM”
At CER, your Recovery Team Consists of:
• Your Community-Based Case Manager
• Your Hawaii Certified Peer Specialist
• Your Registered Nurse
• Your Psychiatrist
• Your Team Leader
• Any other people you designate such as family members or other people who might want to help
